6/5/2023 EnchantaVaughn Approved 1.Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about day. Kaila indicated to QMHPA that she was having a good day overall. QMHPA informed Kaila this session will focus on anger management skills building on anger and how to problem-solve effectively. Kaila did not present any symptoms of S/I or H/I at the time of the session. 2.Behavior: During this second portion of the session, Kaila discussed with QMHPA on how she was a little frustrated with learning how to do everything on her own with 3 children. 3.Behavior: During the third portion of the session, Kaila was optimistic about getting her life back on track to the person she once was before. 1.Intervention: QMHPA inquired about Kaila mental health. QMHPA engaged Kaila in a skills building activity. QMHPA informed Kaila that the purpose of this activity was to model the skills and for her to implement the skills when angry and depressed. QMHPA utilized behavioral skills building activity to allow Kaila to process actions/and or behaviors of others and how it can affect ones daily functioning. QMHPA used communications skills building to reinforce effective communication skills that are assertive communication styles when communicating with others in her home and community. QMHPA conducted a skills building activity with Kaila. QMHPA informed her that the purpose of this activity was to assist her with ways to identify passive and aggressive signs. QMHPA engaged Kaila in a role play activity to assist her with understanding how to utilize these skills when upset. QMHPA engaged Kaila in a discussion surrounding her ADLâs and IDALâs. 2.Intervention: QMHPA engaged Kaila in a skills building activity. QMHPA informed Kaila the purpose of the activity was to assist her with making a chore list of what she needed to do on a daily basis. QMHPA discussed demonstrated with Kaila proper banking techniques on how to save money, cash checks and deposit money. QMHPA engaged Kaila in a skills building activity. QMHPA accompanied Kaila out into the community and taught Kaila the experience how banking works. When QMHPA and Kaila arrived back to the home, QMHPA discussed with Kaila identifying on her schedule the days she washes clothes. QMHPA engaged Kaila in a skills building activity. 3.Intervention: QMHPA informed Kaila that the purpose of this skills was to assist Kaila with reading the calendar and verifying when she needed to do her laundry to keep from the pile that she has sitting in her living room QMHPA encouraged Kaila to become more independent and completing task of her own without the assistance of QMHPA or her any of her family members.to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Kaila struggled with being able to discuss with QMHPA stressors that cause her to become depressed; however, she stressed to QMHPA that she is taking on a lot of tasks that she really should not at her age that is making her depressed. Kaila indicated to QMHPA that she sometimes isolates herself from other people so that she doesnât have to deal with other peopleâs attitudes. Kaila shared that when she is in the community, she feels a little better with handling depression because she is around other people. 1.Response: Kaila appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Kaila expressed to QMHPA that she was in a good mood when inquiring about her overall mental health stability. During the skills building activity, Kaila was able to demonstrate the ability to provide QMHPA with why it was important for her to make her chore list. Kaila verbalized to QMHPA that she sometimes have to be told to complete certain task because she will get lazy. 2.Response: When out in the community, Kaila was able to apply the skill that QMHPA provided her with during the banking process. Kaila acknowledged to QMHPA that she really donât know how to write checks but she uses her debit card a lot. Kaila expressed to QMHPA that she wants to learn how to save more because she thinks that she does spend more than she averages. 3.Response: Kaila discussed with QMHPA on how she is trying not to be depressed on a daily basis. She reports trying to make a way for her and her children. She reports as a single mother it is hard for her to be able to get back on her feet as a single mother. Kaila verbalized to QMHPA on how she is going to make sure she takes full advantage of all the help she can get with the assistance of her mental health worker.
